1. Who May Obtain Death Certificates:

A person or his/her authorized agent having a genealogical, personal, or property right interest in the record.

2. Identification Required:

Driver's License or photo ID. Faxed or emailed photo ID required for online and fax orders, see below.

3. Fee:

$12.00 for the first copy, $4.00 for each additional copy when purchased at the same time. If you request the certificate by mail, payment MUST BE in the form of a money order, cashier's check, or certified check.

Make checks or money orders payable to: CHAMPAIGN COUNTY CLERK. All checks must be numbered 500 or greater. Completed orders can be shipped overnight via Federal Express for an additional cost.

4. Deaths On File In Champaign County:

1878 to present. The Champaign County Clerk's Office has death certificates for only those people who died in Champaign County.

5. How To Obtain Death Certificates:

You may obtain a certified copy, with a raised seal, of a death certificate either in person or by mail. If you are requesting it by mail, you must include the name of the deceased as well as the date of death and a return address along with a copy of photo ID of the person ordering the documents.

*Please note that all fax and online orders for birth, marriage, and death records must be accompanied by a signed, faxed or emailed copy of your photo ID, such as a driver's license. If you do not supply us with the necessary documentation within three (3) business days your order will be cancelled. Our fax number is (217) 384-1241.


Note: Any person who with the intention to deceive, willfully uses or attempts to use any certificate of birth or certified copy of a record of birth knowing that such certificate or certified copy was issued upon a record that is false in whole or in part or that relates to the birth of another person, is guilty of a Class 4 felony in the state of Illinois.
