Birth Certificates - Follow this link to order online*
1. Who May Obtain Birth Certificates:
Child named on certificate, if you are at least 18 years of age or an emancipated minor with court documentation.
Parent, if your name appears on Birth Certificate as mother or father.
Legal Guardian possessing a Court Order stating guardianship.
(Must see original certified court document, will be returned with birth certificate)
Legal Representative representing person on Birth Certificate.
2. Identification Required:
Valid driver's license or acceptable ID. Faxed or emailed copy of identification and written signature required for online orders.* We must see original certified court documents pertaining to any legal matters such as a legal name change or legal guardianship. Documents will be returned when order is processed.
Acceptable Forms of Identification to Obtain Vital Records
One of the following:
Illinois Driver's License
Illinois State Identification Card
Out-Of-State Driver's License or State Identification Card
Passport
US Government Driver's License
Naturalization Certification
US Military Identification Card
US Immigration Card
Selective Service Card

All documents must be current and valid. If the front of a license shows that it is expired, we would also need to see the reverse side with the renewal information.
3. Fee:
$10.00 for the first copy, $2.00 for each additional copy when purchased at the same time.

In Person: Cash, Check (# must be greater than 500), Credit/Debit Card ($1.75 service charge), Money Order, Cashier Check, or Certified Check
By Mail: Money Order, Cashier Check, or Certified Check

Online: Credit/Debit Card ($1.75 service charge)
Online orders can be shipped by Priority Mail or overnight via Federal Express for an additional cost.
4. Births On File In Champaign County:
Only persons born in Champaign County from 1878 to present.
5. To Obtain A Birth Certificate In Person:
To obtain a certified copy, with a raised seal, of your Birth Certificate in person stop by our office at:

6. To Obtain A Birth Certificate By Mail:
To obtain a certified copy, with raised seal, of your Birth Certificate by mail, your request must include a photocopy of your driver's license or acceptable ID, your name at birth, date of birth, parents' full names (mother's maiden name), as well as your written signature, payment and return address.

7.To Obtain A Birth Certificate Online:
To obtain a certified copy, with raised seal, of your Birth Certificate online, you must send a copy of your valid driver's license or acceptable ID and your written signature to us either by fax ((217) 384-1241) or by email (mail@champaigncountyclerk.com) and click here to fill out request form.
*Please note that all online orders for birth, marriage, and death records must be accompanied by a signed, faxed or emailed copy of your valid Driver's License or acceptable ID. Note: Any person who with the intention to deceive, willfully uses or attempts to use any certificate of birth or certified copy of a record of birth knowing that such certificate or certified copy was issued upon a record that is false in whole or in part or that relates to the birth of another person, is guilty of a Class 4 felony in the state of Illinois
