STEP 1
Obtain the proper application for absentee ballot, either by mail or in person, from your election authority (county clerk or board of election commissioners).
STEP 2
Upon receipt, complete the application. Make certain to include your name, home address, the address where you want the ballot to be mailed, and be sure to sign the application. (In primary elections you must state your party affiliation.)
STEP 3
After completing the application, either mail it or deliver it yourself to your election authority. If you return the application in person or complete the application in the election authority’s office, you may immediately vote your absentee ballot in the election authority’s office. If you mail the application and it is properly completed, the election authority will mail your absentee ballot to you.
STEP 4
After receiving your ballot, VOTE THE BALLOT IN SECRET. Insert the ballot into the envelope provided, seal it, complete and sign the certification on the back and PERSONALLY return it or mail it. (The absentee voter may authorize [in writing] a spouse, parent, child, brother, sister, or a licensed motor carrier, to deliver to the election authority the completed absentee ballot in sufficient time to be delivered to the polling place on election day.)
Acceptable Reasons For Voting Absentee
I expect to be absent from my county of residence.
I expect to be temporarily absent from the United States.
I shall be serving as an Election Judge in a precinct which is not my precinct of residence.
I shall be observing a religious holiday.
I shall be performing official election duties for an Election Authority.
I shall be performing election law enforcement duties.
I am physically incapacitated.
I am temporarily abiding in the city, county, state of _____ due to the fact I am a student attending an institution of higher education.
I have been called for Jury Duty on said day.
I am permanently disabled and reside in a Health Care Facility.
I am in the United States Military serving active duty and expect to be absent from the county of residence.
Requesting An Application For Absentee Ballot
Absentee voting may be done by individuals who are unable to make it to the polling place on the day of election, yet still wish to exercise their right to vote.
Absentee voting may be completed by three different methods:
1. You may request an absentee ballot through the mail.
2. You may request an absentee ballot online - follow this link to the form.
3. You may come into the County Clerk's Office and vote absentee here.

NOTE: The following information is needed when requesting an application for absentee ballot. We need to know your full name, your local address and the address where you need the application and ballot sent.
The application for absentee ballot must be returned to us via mail or by the person whose name appears on the application. We cannot accept this application if it is returned by anyone other than the person whose name appears on the application.
Voting Absentee At The County Clerk's Office
You may stop by our office located at 1776 E. Washington in Urbana between the hours of 8:00 A.M. and 4:30 P.M. We are open Monday through Friday except for County Holidays.
An absentee applicant's identity must be verified by presentation of an IL driver's license, IL ID card, or another government issued ID containing the applicant's photograph. If the person is a first time voter required to show ID, the acceptable forms of ID include: a current and valid photo ID, a current utility bill, bank statement, government check, paycheck, or other government document that shows name and address. A voter registration card is not acceptable identification.
Military and Overseas Absentee Voting
If you are in the military or overseas the best way for you to request an absentee ballot is to send us an FPCA form. The FPCA form is a red and white form that serves as a request for absentee ballot. If you are in the military you can ask your commanding officer for the form. If you are overseas you may request a FPCA form from the U.S. Embassy or Consulate. If you are unable to obtain the FPCA form please notify our office as soon as you can so we can mail you an application for absentee voting.

Permanently Incapacitated Voter Program
This program is for those people who are permanently incapacitated.
To be put on this program you need to contact the County Clerk's Office.
You will then be given a form that has to be filled out. There is a section that has to be filled out by (or for) the incapacitated voter and a section that must be completed by their doctor.
This program will then allow the County Clerk's Office to automatically send the person an application for absentee ballot before every election (that they qualify to vote in) for a five year period. After this the form must be renewed.
Temporarily Absent Student Voter Program
If you are a student attending an institution of higher learning then you can enroll in the temporarily absent student voter program.
You can dowload the Temporarily Absent Student Notice form, fill it out, and mail it to our office OR you can come into the County Clerk's Office and pick up a Temporarily Absent Student Notice.
Once the form is filled out then you will receive an application for absentee voting before each election for a two year period.
Note: Students are responsible for notifying the County Clerk's Office of any changes in their college mailing address.

Most first time voters who have registered to vote by mail must vote in person, either in person absentee voting (at the County Clerk's Office), as a hospitalized or nursing home voter, or at the precinct polling place.
The first day to receive an application for absentee ballot through the mail or to vote in person at the County Clerk's Office is approximately 40 days prior to the election. If you are not sure if absentee voting is taking place please call our office at (217) 384-3724.
The last day that absentee ballots may be mailed out is 5 days prior to the election. This is also the last day that we may receive an application by mail from anyone who has requested an absentee ballot.
